In order to discuss the biochemical diagnosis of hypogonadism it is necessary to outline the usual carriage of testosterone in the blood. Total serum testosterone consists of free testosterone (2%–3%), testosterone bound to sex hormone binding globulin (SHBG) (45%) and testosterone bound to other proteins (mainly albumin −50%) (Dunn et al 1981). Testosterone binds only loosely to albumin and so this testosterone as well as free testosterone is available to tissues and is termed bioavailable testosterone. Testosterone bound to SHBG is tightly bound and is biologically inactive. Bioavailable and free testosterone are known to correlate better than total testosterone with clinical sequelae of androgenization such as bone mineral density and muscle strength (Khosla et al 1998; Roy et al 2002). There is diurnal variation in serum testosterone levels with peak levels seen in the morning following sleep, which can be maintained into the seventh decade (Diver et al 2003). Samples should always be taken in the morning before 11 am to allow for standardization.
This study [9] also reported significantly increased glutathione levels. Glutathion has been shown to have a synergetic effect with l-citrulline as their combination further increases nitrate and nitrite levels and contributes to the sustained release of NO. While some previous studies have indicated that glutathione stimulates L-arginine turnover and increases nitric oxide synthase (NOS).
Andy – If you take testosterone, your hypothalamus will notice an abundance of testosterone in your system and will instruct your pituitary gland not to send LH and FSH to your testicles. If you’re testicles are not receiving these two hormones, they will stop making testosterone. If you quit testosterone call turkey, you will likely fall back to your baseline level fairly quickly. There is a protocol however to get back to your normal production much quicker should you choose to quit therapy.

I do not intend to spill my guts about symptoms or values on here as it has already been done so many times over. I will say that I am an RN BSN and have been on HRT for going on 6 yrs. I live in Oklahoma and when I first started noticing symptoms I knew something was wrong and began to do research myself. With an initial level of 241 no doctor here at that time, would even consider HRT as I was only 35ish at the time.

After 90 days, it seemed like the Andro Gel kind of stopped having it’s effect. Went back to see the doc, and my levels had dropped back to about 198 from high 200’s. Doc said it’s not unusual that the body gets used to it, and maybe I needed to adjust up. I switched to four pumps per day, and I felt immediate effects. Now…there is a period of ultra-horniness, and sometimes, you will feel some of the “roid rage” at some of these levels. It varies from person to person. Generally, if you are an ass, or high strung, this will amplify it. I started testosterone therapy in January 25th 2014. Original Levels 206 total and 3.5 free. Now I am 1350 total and 125 free. I can honestly say I have felt no different. Still just as tired, Sexual interest is very high as it was before therapy. I am over weight with a fat belly. My biggest complaint is feeling tired. That has not changed. My weight increased a .little since beginning therapy. Who knows??

There are a number of companies that are selling “legal pro-hormones” or “test boosters”. I used several types. They were banned several times, and re-introduced, and ultimately have no real FDA oversight. Specifically LG Sciences, and a couple of others I tried. They do work, you will see gains, and quickly. You will gain a lot of water weight. You will get angry. You will eventually damage your liver no matter what their advertising tells you. I had a general physical, and my doctor thought I was an alcoholic…when I do not drink, he was shocked. I’ve flushed it all down the toilet, and the doc said to trim down the powdered supplements as well, as they can be very harmful. So….please heed my warning, that stuff will hurt you.

Recently, a panel with cooperation from international andrology and urology societies, published specific recommendations with regard to the diagnosis of Late-onset Hypogonadism (Nieschlag et al 2005). These are summarized in the following text. It is advised that at least two serum testosterone measurements, taken before 11 am on different mornings, are necessary to confirm the diagnosis. The second sample should also include measurement of gonadotrophin and prolactin levels, which may indicate the need for further investigations for pituitary disease. Patients with serum total testosterone consistently below 8 nmol/l invariably demonstrate the clinical syndrome of hypogonadism and are likely to benefit from treatment. Patients with serum total testosterone in the range 8–12 nmol/l often have symptoms attributable to hypogonadism and it may be decided to offer either a clinical trial of testosterone treatment or to make further efforts to define serum bioavailable or free testosterone and then reconsider treatment. Patients with serum total testosterone persistently above 12 nmol/l do not have hypogonadism and symptoms are likely to be due to other disease states or ageing per se so testosterone treatment is not indicated.

The fact that fenugreek increases libido is not mythology; it is backed by a clinical study which showed the libido of men aged 25-52 increased by 25%[1] on average when taking fenugreek extract for six weeks.  According to Lee Myer, of www.peaktestosterone.com fame, fenugreek will help you achieve orgasm as well, so if this is an issue for you, fenugreek may be the answer.  It’s a frustrating problem to have.

Let’s first start off by saying, there is no comparison of natural testosterone boosters to synthetic.  When referring to synthetic, we’re talking anabolic steroids.  At that point, you’re comparing apples and oranges.  Which is best—natural or synthetic?  Synthetic.  It is much easier to put on quality muscle mass while using anabolics than it is using a natural testosterone booster.  One is basically giving you synthetic testosterone while the other is giving you a minor boost in natural testosterone production.  However, not only are there side effects to the use of anabolics, but they are also illegal in the United States unless used under a doctor’s supervision with a prescription.
The researchers found that the dose of testosterone required to produce different effects in the body varied widely. The influence of testosterone and estradiol also differed. As the testosterone gel dose was reduced, the scientists showed, reductions in lean mass, muscle size, and leg-press strength resulted from decreases in testosterone itself.
